Which are we saving - lives or money?

My pager sometimes would beep at 3:00 am and as usual I only had 20 minutes to get to the hospital and be ready to receive a patient with an ongoing heart attack. The 20 minutes is an established protocol proven to be the acceptable time in order to save heart muscle.
